  • Change-of-Address Form - Important : All name/address changes or corrections must be submitted in writing. Please download the above Change-of-Address Form for changes or corrections form. The form is a fillable PDF format. Any changes or corrections are to be mailed or faxed ONLY, to the Board of Cosmetology. Please allow 48 hours for changes to be reflected in the system before continuing your Cosmetology License online renewal.
