I. APPROVAL OF MINUTES: June 18, 2008 - Approved as amended.

II. EXECUTIVE DIRECTOR’S REPORT:

1. Dr. Bisma Sohail - The Board discussed this application for a resident permit. The Board approved this application pending receipt of the official transcripts.

2. The Board requested that CODA be contacted to obtain a list of approved residency Programs in New Jersey.

3. Dr. Eli Eliav - The Board discussed this application for limited teaching certificate. Dr. Eliav is a graduate of an unapproved dental school. The Board in a motion by Dr. Hall, seconded by Dr. Dolinsky, approved this application indicating that Dr. Eliav is immensely qualified. Dr. Eliav must comply with the statutory limitations on practice. The Board noted he would not be permitted to participate in faculty practice.

III. COMMITTEE REPORTS:

1. Credentials:

Dental:

- Dr. Jude A. Thomas - The committee discussed a request from Dr. Thomas questioning whether he qualifies for a New Jersey dental license. He took the NERB exam in 1996. He practiced dentistry in Pennsylvania from 1999 to 2005. His Pennsylvania license has been inactive since 2005 and he has been practicing law. The committee recommended that he be advised that he could apply for a New Jersey dental license.

- Dr. Michael Shyong - Application for a dental license. Dr. Shyong meets all requirements for licensure. The committee recommends approval.

- Dr. Joshua John Bloch - Application for a dental license. The committee recommended that additional information be requested from Dr. Bloch concerning information disclosed on his application.

Dental Auxiliaries:

- Jessica Eureyecko - Application for a dental hygiene license. Ms. Eureyecko meets all requirements for licensure. The committee recommends approval.

- Betty V. Fernandez - Inquiry concerning specific duties of a dental hygienist. The Board determined that Ms. Fernandez be advised that in order to provide her with a response, additional information must be provided concerning the incident.

The Board in a motion by Ms. Delaney, seconded by Dr. DeSciscio, approved the committee’s recommendations.

2. Continuing Education:

- Dr. Henno Prima - The Board discussed Dr. Prima’s request for extension to complete the continuing education credits per the Board’s Consent Order. The Board granted an extension until September 30, 2008.

- The Board discussed a request from Janice Gibbs, New Jersey Dental School, for approval of "Removable Prosthodontics Remediation Course." The Board approved this course.

- Jersey Shore Medical Center - The Board discussed an application for approval of enteral sedation course for the renewal of the enteral sedation permit. The Board approved this application.

IV. APPEARANCES:

1. 9:30 A.M. - Allen Koral, D.D.S. - Appearance concerning information disclosed on his application for a dental resident permit.

This matter was adjourned. Dr. Koral is no longer applying for a dental resident permit.
