Notice Regarding Applications

To the extent possible, please have the following application documents sent to the board via email,
occupationaltherapy@dca.lps.state.nj.us
- All official transcripts sent directly from the school
- Verification of Field Work Experience sent directly from the school
- Certicate of Good Morale Character Form
- All official license verifications sent directly from the state that has issued the license
- Any name change documents
To
check the status of your application, please to go to
NJConsumerAffairs.gov/Pages/licensingboards.aspx - Click #4 “Check Status of an Aplication” (This will allow you to view the status of your pending application online.).
- Use the same log in and password you created when you completed the online application.
- You may now view items as they are received, and checked off on your pending application.
