On January 8, 2024, Governor Murphy signed into law P.L. 2023, c. 237, which, among other things: amended the Contractors’ Business Registration Act (“CBRA,” formerly the “Contractors’ Registration Act”), N.J.S.A. 56:8-136 et seq., and created the “Home Improvement and Home Elevation Contractor Licensing Act,” N.J.S.A. 45:5AAA-1 et seq. On July 10, 2024, Governor Murphy signed into law the Real Estate Consumer Protection Enhancement Act, P.L. 2024, c.32, which, among other things, requires sellers of residential property located in New Jersey to use the "Seller's Property Condition Disclosure Statement" ("Disclosure Statement," questions 1 through 108).

Additionally, on July 3, 2023, Governor Murphy signed into law P.L. 2023, c.93, which, among other things, requires sellers of all real property located in New Jersey to make certain additional disclosures concerning flood risks on the "Disclosure Statement." On July 15, 2024, the Division published a "Flood Risk Addendum" to the Disclosure Statement (questions 109 through 117), which includes the additional disclosures concerning flood risks.

As a result of these two laws, effective August 1, 2024:
  • Sellers of residential property must complete the Disclosure Statement (questions 1 through 108). A copy of the Disclosure Statement is available here; and
  • All sellers of real property, both residential and non-residential, must complete the Flood Risk Addendum to the Disclosure Statement (questions 109 through 117). A copy of the Flood Risk Addendum is available here.

The Division has created an instruction sheet with additional information regarding the use of these forms. The forms linked above supersede any forms previously posted by the Division, including, but not limited to, the "Amended Disclosure Statement" posted on December 21, 2023.

Please Read: Paper applications are no longer accepted. Any paper applications received, will be returned to you and you will be redirected to complete an application online. 


Reinstatements/ Reactivations (All License Types)

  1. Fill out the Application for Reinstatement/Reactivation

  2. Fill out Attestation in Lieu of Notary

  3. Upload all documents to your online profile through the Upload License Documents tab

  4. All third party documents including official transcripts, license/certificate verifications are to be emailed to DCA_Social_Work_No_Reply@dca.njoag.gov.

  5. Staff will email you instructions to pay application fee(s).

  6. Inquiries about the application process can be sent to swReinstatement@dca.njoag.gov.


 

Application for Board Acceptance as a Continuing Education Approval Entity

  1. Fill out the Application for Board Acceptance as a Continuing Education Approval Entity.

  2. Email completed application to DCA_Social_Work_No_Reply@dca.njoag.gov.

  3. Staff will email you instructions to pay application fee.

  4. Inquiries about the application process can be sent to SocialWork@dca.njoag.gov.


 

Name Change Request

  1. Fill out a Change of Name Form.

  2. Scan completed form and all supporting documents to your online profile through the Upload License Documents tab.

  3. If requesting a duplicate license you will be emailed an invoice for $25 and instructions to pay.

  4. Follow up inquiries may be sent to SocialWork@dca.njoag.gov.


 

License Verification Requests

To request a verification of your New Jersey license status to another state, reach out to the requesting board/jurisdiction and obtain an email address to which we can send electronic verification. We can only provide electronic verification at the time.

Once you've obtained this, send an email with the information below to: SocialLV@dca.njoag.gov.

Your request must include:

  1. Your New Jersey license/certificate number

  2. Your mailing address

  3. Your e-mail address

  4. The e-mail address where verification is to be sent.

Once we receive your email request, we will reply with an invoice for the $25 verification fee (which you can pay online to expedite processing). Once you pay your invoice, reply to the email chain with a copy of your receipt. Once we receive this and confirm your payment, we will issue the license verification to the email address of the State licensing Board you've provided.

Please note: incomplete requests will delay the processing of the verification.

The Board only provides verification of information for which they are the primary source. This means that if you submitted the information with your application (such as clinical experience forms) we cannot specifically confirm the veracity of this information in verification to another Board.

We will, however, digitally forward copies of your submitted documentation to licensure Boards in other states. Please mention that you would like your original supervision documentation forwarded in the verification request you email to us. We will email the supervision documentation we have on file directly to the Board of your choice. PLEASE NOTE: we will forward documentation to your designated Board as soon as possible, but understand that older documentation may take longer to obtain and forward.


 

Proposed Plan of Supervised Clinical Experience

  1. Fill out a Proposed Plan of Supervised Clinical Experience.

  2. Upload completed application and all supporting documents to the online profile of the LSW through the Upload License Documents tab.

  3. There is no fee for this application.

  4. All subsequent inquiries should be sent to SocialWork@dca.njoag.gov.

The proposed plan of supervision can be submitted prior to the start date or while the supervision is ongoing. The purpose of a proposed plan is to allow the Board to approve/deny/offer feedback to the LSW about clinical supervision before the hours have been completed.

 

Duplicate License Request

  1. Fill out a Duplicate License Form.

  2. Upload completed form to your online profile through the Upload License Documents tab.

  3. You will be emailed an invoice for $25 and instructions for payment.

  4. All subsequent inquiries should be sent to SocialWork@dca.njoag.gov.


 

To Check the Status of Your Application

  1. Log into your online profile.

  2. On the application type you wish to see click on View Checklist.

  3. Items that do not have a complete date next to them are still missing.

  4. You may upload missing items to your online profile through the Upload License Documents tab except for third party documents (transcripts, license verifications) which should be received directly by the Board from those entities and are to be emailed to DCA_Social_Work_No_Reply@dca.njoag.gov.
