  ​Notice Regarding Applications

  ​ To check the status of your application, please to go to NJConsumerAffairs.gov/Pages/licensingboards.aspx  Click #4 “Check Status of an Aplication” (This will allow you to view the status of your pending application online.). Use the same log in and password you created when you completed the online application.  You may now view items as they are received, and checked off on your pending application.

  Initial form for individuals or entities that employ another person to engage in, or that employ another and advertise or hold themselves out as providing, massage and bodywork therapy.

Reinstatements / Reactivations

• Application for Reinstatement or Reactivation of a License
  You are required to submit proof of your professional liability insurance along with this application.
  
  

  On the employment history page please be sure to provide the following:

  A detailed employment history to include name(s) of employer(s), address of employer(s), dates of employment, job title(s), job duties and hours worked for your application to be reviewed.

  If you do not have previous employment history, provide a narrative explaining what you were doing from the time the license expired to the time you filed the application.

  Please email your reinstatement application to massagetherapy@dca.njoag.gov for immediate review.  Do not mail the reinstatement application or a payment, we will email you an invoice to pay online once you we receive your reinstatement application via email. Thank you.
  
  

• Application to Reinstate Business License
  ONLY complete this application if you have previously had a Business License that is currently expired. If you are applying for a Business License for the first time, please complete the Massage and Bodywork Therapist Employer Registration  form in the above section. ​
  
  Please email  a completed reinstatement application to massagetherapy@dca.lps.state.nj.us – do not mail a payment, we will email you an invoice to pay online once you email your application to the board office.  Thank you.
  

Certifications

• Supervising Faculty Member Certification
  To the sending school - please be advised, even if you are signing for a closed school, you must complete the information in the form of the applicant name, the school and the school address that the applicant attended. If you sign only the bottom of the form, the form cannot be accepted.
