To assist your business in complying with the Kosher Food Consumer Protection Act, N.J.S.A. 56:8-61 et seq., you should review the relevant statute and regulations
(links below), and request and complete the applicable Disclosure Form(s) and Disclosure Statement(s) from the Division of Consumer Affairs.
NOTE:
Depending on your business practices, you may require more than one Disclosure Form and Disclosure Statement.
To request the applicable Disclosure Form(s) and Disclosure Statement(s),
contact the Bureau of Kosher Enforcement. In your email, specify all of the applicable practices that apply to your business:
- Bakery,
- Meat,
- Meat, Dairy and Pareve, and/or
- Passover.
Complete the
Disclosure Form(s) and Disclosure Statement(s) with the exact same information after you receive them from the Division of Consumer Affairs. On the back of the Disclosure Form(s), list your business name, personal name and title, business address, date, and signature. Return the Disclosure Form(s) to the Division in the self-address envelope provided within 14 calendar days of receipt.
You are required to place the Disclosure Statement(s) in a readily visible location in your business premises.
If any of the information in your Disclosure Statement(s) changes or if additional information needs to be added after submission of your completed Disclosure Form(s), you must immediately amend your posted Disclosure Statement(s) and send a new Disclosure Form(s) to the Division within 14 calendar days.
If an additional Disclosure Statement applies to your business
(e.g., adding Bakery to Meat), you must immediately contact the Division to obtain the appropriate Disclosure Form and Statement.
