Reinstatements/ Reactivations (All License Types)

An individual whose license or certificate has been automatically suspended for nonpayment of a biennial renewal fee, or has been on inactive status, and wishes to be reinstated or reactivated by the Board should consult N.J.A.C. 1344G-7.

How to apply for reinstatement or reactivation by the Board:

  1. Fill out the Application for Reinstatement/Reactivation
  2. Fill out Attestation in Lieu of Notary
  3. Upload all documents to your online profile through the Upload License Documents tab
  4. All third party documents including official transcripts, license/certificate verifications are to be emailed to DCA_Social_Work_No_Reply@dca.njoag.gov
  5. Staff will email you instructions to pay application fee(s)
  6. Inquiries about the application process can be sent to swReinstatement@dca.njoag.gov




Continuing Education

Formally Recognized Entities

The Board Adopted Amendments: N.J.A.C. 13:44G-6.3, 6.4, and 6.7 Content Areas for Continuing Education Credit on April 15, 2024. This updates the Board's regulation. 

The update allows that attendance at programs or courses offered by providers approved by the National Association of Social Workers (NASW) or state chapters of the NASW: or attendance at programs or courses offered by providers approved by the Association of Social Work Boards (ASWB) are now acceptable sources of CE credit.

At the June 12, 2024 public meeting, the Board voted to allow "any applications submitted, or audits which take place, on or after September 1, 2022"  to be able  "to submit CE from the sources updated in the new amendment adopted on April 15, 2024."

The Board noted that individuals should review all regulations related to CE to ensure CE coursework is in compliance with all the related regulations. 

Please consult N.J.A.C. 13:44G-6 for more information on Continuing Education (CE) regulations, including alternate sources of CEs. It is the responsibility of Board licensees and certificate holders to read and interpret these regulations.

Board-approved Postgraduate Coursework Related to Clinical Supervision

Beginning on July 7, 2004, clinical supervision shall be rendered by an LCSW with a minimum of three years of licensure as an LCSW and who has completed at least 20 continuing education credits of post-graduate course work related to (clinical) supervision. The following coursework was reviewed by the Board and is Approved to meet the requirements according to N.J.A.C. 13:44 G-8.1(a)4:

Application for Board Acceptance as a Continuing Education Approval Entity

An entity that wishes to obtain Board recognition as an entity that may approve continuing education courses shall submit the approval entity applicant fee set forth at N.J.A.C. 13:44G 14.1(a)16 and apply to the Board for such recognition in writing. Please see at N.J.A.C. 13:44G-6.7 for details on the requirements of these programs.

To apply for Board recognition:

  1. Fill out the Application for Board Acceptance as a Continuing Education Approval Entity
  2. Email completed application to DCA_Social_Work_No_Reply@dca.njoag.gov
  3. Staff will email you instructions to pay application fee
  4. Inquiries about the application process can be sent to SocialWork@dca.njoag.gov




Other Forms and Instructions

Name Change Request Form

  1. Fill out a Change of Name Form
  2. Scan completed form and all supporting documents to your online profile through the Upload License Documents tab
  3. Make sure to choose "Name Change Document" from the "Document Type" drop-down menu
  4. If requesting a duplicate license you will be emailed an invoice for $25 and instructions to pay
  5. Follow up inquiries may be sent to SocialWork@dca.njoag.gov


Duplicate License Request Form

  1. Fill out a Duplicate License Form
  2. Save the file name as "(Your Last Name) Duplicate License Form"
  3. Upload completed form to your online profile through the Upload License Documents tab
  4. You will be emailed an invoice for $25 and instructions for payment
  5. All subsequent inquiries should be sent to SocialWork@dca.njoag.gov

 

License Verification Request Procedure

To request a verification of your New Jersey license status to another state, reach out to the requesting board/jurisdiction and obtain an email address to which we can send electronic verification. We can only provide electronic verification at the time.

Once you've obtained their email address, send an email with the information below to: SocialLV@dca.njoag.gov.

Your emailed request must include:

  1. Your New Jersey license/certificate number
  2. Your mailing address
  3. Your e-mail address
  4. The e-mail address where verification is to be sent.

Once we receive your email request, we will reply with an invoice for the $25 verification fee (which you can log into your EGov account to pay online to expedite processing). Once you pay your invoice, reply to the email chain with a copy of your receipt. Once we receive this and confirm your payment, we will issue the license verification to the email address of the State licensing Board you've provided.

Please note: incomplete requests will delay the processing of the verification.

The Board only provides verification of information for which they are the primary source. This means that if you submitted the information with your application (such as clinical experience forms) we cannot specifically confirm the veracity of this information in verification to another Board.

We will, however, digitally forward copies of your submitted documentation to licensure Boards in other states. Please mention that you would like your original supervision documentation forwarded in the verification request you email to us. We will email the supervision documentation we have on file directly to the Board of your choice. PLEASE NOTE: we will forward documentation to your designated Board as soon as possible, but understand that older documentation may take longer to obtain and forward.


To Check the Status of Your Application

  1. Log into your online profile
  2. On the application type you wish to see click on View Checklist
  3. Items that do not have a complete date next to them are either missing, incomplete, or under review
  4. You may upload missing items to your online profile through the Upload License Documents tab except for third party documents (transcripts, license verifications) which should be received directly by the Board from those entities and are to be emailed to DCA_Social_Work_No_Reply@dca.njoag.gov

If you have uploaded an item for review and it has been more than 4 weeks since the date of your upload, you can submit an inquiry about the status of your application to SocialWork@dca.njoag.gov. Please note, an application status would not be available prior to the estimated timeframes provided. 




Exam Extension Request Procedure

  1. Send an email to: SWExam@dca.njoag.gov
  2. Your email must contain the following information:
    1. Your full first and last name as they appear on your application.
    2. The name of the exam you are requesting an extension on (ASWB Masters or ASWB Clinical).
  3. Our staff will review your file and determine if any additional information is needed to approve the request.
  4. Submitting this request to a different email address than SWExam@dca.njoag.gov will result in delays.
